Thoracic Outlet Syndrome 

may be the result of an anomalous cervical rib
that compresses the lower trunk of the brachial plexus, subclavian artery, or both.

Clinical findings include
atrophy of thenar and hypothenar eminences, atrophy of interosseous muscles, sensory deficits on the medial side of forearm and hand, diminished radial artery pulse upon moving the head to the opposite side, and a bruit over the subclavian artery. The angiogram, taken with abduction of both arms, shows that blood flow is partially occluded in the subclavian arteries (arrows).

ورم مخاطي الاذني

مرض حميدي💜,في اغلب الاوقات

يظهر في جدار العضلة القلب ثم يمتد الى جوف القلب ، ثم يستقر في الأغلب الاذين الايسر.
يكون من مادة "جلاتينية هولامية"

الأسباب:
غير مؤكدة حاليا لكن يعتقد البعض انه بسبب فرط نمو خلايا خلايا القلب.🚶
% من هذه الاورام المخاطية هي ذات صبغة وراثية منقولة من الاباء والامهات إلى الأبناء بالصبغية الجسدية السائدة (Autosomal dominant)، ولهذا يطلق على هذا الورم اسم "الورم المخاطي العائلي" (Familial myxoma) .✋
1-انفصال مركب من الورم.
2-خثرة .
3-انصمام ( embolism).
4-اعاقة تدفق الدم في القلب.
5-ضيق تنفس.
6- وذمة .
7- دوخة أو اغماء.
8-حمى .
9-انخفاض الوزن.

علاج :
هو عملية جراحية لاستئصال الورم
يكون علاج هذا المرض عن طريق الجراحة حيث يتم استئصال الورم المخاطي الأذيني اليسار شق المتنوعة، وقطع الأذين الأيمن تحت المجازة القلبية الرئوية وإزالة الورم، مع إصلاح ألياف البوليستر في الحاجز الأذيني .💙

الاسترواح الصدري (Pneumothorax)

هو ظاهرة تتمثل بتراكم الهواء بين أنسجة الرئة وجدار الصدر، وهو معروف منذ العصور القديمة. يحيط بالرئة غلاف مكوّنٌ من طبقتين: طبقة ملتصقة بالرئة - هي الغشاء البلوري الداخلي (visceral pleura)، وطبقة أخرى ملتصقة بالسطح الداخلي للقفص الصدري - تسمى بالغشاء البلوري الخارجي (parietal pleura). عند التنفس بشكل عادي، يلتصق الغلافان ببعضهما، ويخرج الهواء الذي نستنشقة عبر الزفير. أما في حالة الإصابة بالاسترواح الصدري، فيقوم الهواء المتراكم بالفصل بين طبقتي الغلاف.

أسباب وعوامل خطر الاسترواح

من الممكن أن تحدث الإصابة بالاسترواح الصدري المكتسب خلال تلقي علاج طبي بسبب حدوث ثقب بغلاف الرئة، أو عند التعرض لإصابة حادة في الصدر نتيجة لحادث سيارة أو للسقوط من ارتفاع كبير، وكذلك نتيجة للإصابات التي تخترق الصدر كالإصابة بطعنة سكين أو بعيار ناري على سبيل المثال.

أما الاسترواح الصدري الذاتي، فهو عادة ما يكون نتيجة لمرض رئوي، مثل مرض الانسداد الرئوي المزمن (COPD - Chronic Obstructive Pulmonary Disease), الربو (Asthma), والتليف الكيسي (Cystic Fibrosis). وفي بعض الحالات النادرة، يكون ناجما عن الإصابة بسرطان رئوي أو بنقائل سرطانية في الرئتين (metastases)، خاصة سرطان السركوما (Sarcoma).

العامل الذاتي الأكثر انتشارا، الذي يسبب الإصابة بالاسترواح الصدري, هو استرواح الصدر العفوي الابتدائي (Primary spontaneous pneumothorax). يكون أغلب المصابين به (بشكل غير واضح تماما) شباب نحيفون وطوال القامة، في سنوات العشرين من العمر، وهم عادةً ما يعانون من آلام مفاجئة في الصدر، مصحوبة بضيق في التَّنَفُّس، حتى دون القيام بأي مجهود جسدي شاق. وقد بيّنَت بعض الأبحاث أن هنالك احتمالا لدى هذه الفئة من الأشخاص لتشكّل الفقاعات (Bleb) في الجزء العلوي من الرئة، لكن سبب ظهور هذه الفقاعات أو وظيفتها ما زال غير واضح. الفرضية السائدة هي أن هنالك علاقة للالتهاب بمجرى الهواء. سبب حدوث الاسترواح الصدري الذاتي هو تمزق إحدى الفقاعات الموجودة على غشاء الرئة الداخلي (visceral pleura). يسمح تمزق غشاء الرئة بمرور الهواء بين داخل الرئة وبين الأغشية التي تغلفها، ويتيح تراكم الهواء بينهما.

تظهر الدراسات أن التدخين يزيد من وتيرة وسرعة تمزق الفقاعة.

أحد العوامل الذاتية الإضافية التي تؤدي لحدوث الاسترواح الصدري، هو الإصابة بمرض الانسداد الرئوي المزمن (Chronic obstructive pulmonary disease - COPD). يتميز هذا المرض بالتوسع المرضي لفراغات الهواء الصغيرة الموجودة في الرئه، والمعروفة باسم الفقاعات (Bullae). ينشأ الاسترواح الصدري بسبب تمزُّق إحدى الفقاعات (Bullae). ويؤدي الاسترواح الصدري العفوي لتفاقم أعراض (بالاساس ضيق التنفس) أمراض الرئتين التي أدت إلى حدوثه.

ينشأ الاسترواح الصدري المكتسب نتيجة لإصابة خارجية أدت لحصول ثقب أو تمزق بغشاء الرئة.

عادة ما تكون الأعراض التي تميز الاسترواح الصدري العفوي أو المكتسب متشابهه. عند حدوث ثقب في غشاء الرئة، فإن الهواء يتراكم حول الرئة ويؤدي للضغط عليها، وحتى لحصول انهيار جزئي فيها. نتيجة لهذا الانهيار، ينغلق الثقب وتتوقف عملية دخول الهواء إلى الفراغ بين طبقتي الغشاء البلوري للرئة (pleura).

تشخيص الاسترواح الصدري

يمكن تشخيص الاسترواح الصدري بسهوله نسبية، وذلك بواسطة التصوير الإشعاعي للقفص الصدري.

علاج الاسترواح الصدري

إذا كانت حدّة الإصابة بالاسترواح الصدري منخفضة لدى أشخاص لا يعانون من أية خلفية مرضية، فإن علاج الاسترواح الصدري يرتكز بالأساس على إخضاع المريض للإشراف الطبي والمتابعة فقط، وذلك من خلال التصوير الدوري للقفص الصدري، إلى أن يتم تفريغ كل الهواء الموجود في الفراغ الذي بين طبقتي الغشاء البلوري (pleura)، وتوسّع الرئتين بشكل كامل. أما إذا لم تتوسع الرئتان, أو عندما يكون الاسترواح الصدري أكثر حِدّة، فيتم إدخال أنبوب وَرْبيّ للتفريغ (intercostal drain) تحت التخدير الموضعي.

يمكن أيضا القيام بالعلاج بواسطة جراحة بضع الصدر (Thoracotomy).

من المهم أن نعرف أن الاسترواح الصدري الذاتي الأولي قد يتكرر. ويصل معدل تكراره إلى نحو 50% من الحالات.

في السنوات الاخيرة، أخذ العلاج بواسطة منظارالصدر (Thoroscopy) بالانتشار كعلاج جراحي للاسترواح الصدري.

يفوق معدل ظهور الاسترواح الصدري مجددا (التكرار) بعد الخضوع للجراحة التنظيرية في الصدر (Thoroscopy)، معدل ظهوره مجددا بعد الخضوع لجراحة بضع الصدر (Thoracotomy) بقليل. غير أنه من المتبع إجراء الجراحة التنظيرية للصدر نظرا لكونها إجراءً علاجيا أقل جورا من نظيرتها التي تعتمد بضع (شق) الصدر.

Meningitis is inflammation in the meninges, which protect the brain and spinal cord. Meningitis is most often caused by a virus or bacteria.

The most common symptoms of meningitis in babies include:
1- Bulging fontanel (the soft spot on top of the head). This may be due to increased pressure or fluid in the brain.
2- Fever. A high temperature is a red flag for an infection, but some babies, especially those under 3 months of age, may not have a fever.
3- Cold hands and feet with a warm torso.
4- Chills. This may include shivering or chills, with or without a fever.
5- A stiff neck. Babies may hold their bodies in a stiff position and may hold their head tilted back.
6- Irritability and crying, especially when picked up. This could be due to a sore or stiff neck or muscle and body aches.
7- Rapid breathing.
8- Vomiting persistently.
9- Refusing to feed.
10- Extreme sleepiness. A person may have difficulty with or be unable to wake the baby.
12-Red or dark rash or marks on the body

Syncop:
It is helpful to think about the pathophysiology of vasovagal syncope as a reflex arc, with an afferent limb and an efferent limb. Although the autonomic nervous system mediates vasovagal syncope,  pathophysiological mechanisms are not completely understood. The afferent limb of the reflex arc begins with a trigger. Although this trigger may be emotional stress or pain, it is often unidentifiable. It is believed that this trigger, usually in combination with central hypovolemia (from upright posture or dehydration) results in increased cardiac contractility in the setting of a relatively underfilled left ventricle. This may trigger mechanoreceptors in the ventricle that signal via vagal afferents to the central nervous system.
The efferent limb of the reflex arc is better understood. Increased vagal firing (increased parasympathetic activity) at the sinus node and the atrioventricular node causes a decrease in heart rate. This decrease in heart rate can be profound, with asystole that can be several seconds long. At the same time, decreased sympathetic activity results in decreased vascular tone in both arterioles and venules. The results in decreased preload, venous return, and ventricular volume. Since cardiac output is the product of stroke volume and heart rate, this reflex arc affects both factors in the equation: slowing the heart rate and decreasing the amount of volume. This results in a drop in the patient's mean arterial pressure. Cerebral autoregulation results in constant cerebral blood flow over a wide range of mean arterial pressures, but when the mean arterial pressure falls below the body's ability to autoregulate, the patient loses consciousness. Typically, if the patient falls or is laid supine, the increase in circulating blood volume from the lower extremities combined with the decreased work necessary to get blood to the brain (i.e., the heart does not have to pump "uphill" against gravity) will cause the patient to regain consciousness rapidly.

The following diagram shows the anterior chest again, with the lobes of the lungs included. Various reference lines and angles are commonly used to identify respiratory findings. For example:
1_The angle of Louis (also called the sternal angle) is a useful place to start counting ribs, which helps localize a respiratory finding horizontally.
If you find the sternal notch, walk your fingers down the manubrium a few centimeters until you feel a distinct bony ridge.
This is the sternal angle. The 2nd rib is continuous with the sternal angle; slide your finger down to localize the 2nd intercostal space. The angle of Louis also marks the site of bifurcation of the trachea into the right and left main bronchi and corresponds with the upper border of the atria of the heart.

2_Reference lines help pinpoint findings vertically. For example, the major division ("fissure") between lobes in the anterior chest crosses the 5th rib in midaxillary line and terminates at the 6th rib in the midclavicular line.
