Clarity on Forms & Resources from the COVID pack
The idea is to action and utilise as many of the resources listed on the pack as personally possible. Even if the ideal outcome is not initially achieved, they all serve a purpose in our advocacy action plan.
Ideally the forms presented to your GP, are signed by them, but even if they don't oblige, it still serves as a documentation purpose to have it recorded on your file (so you can leave a copy with them and take a copy for yourself - making a note to record the date it was presented to your GP).
There are also two exemption forms (linked on the pack); one of which is a self declared exemption.
The exemption, declination, and open letter can all be utilised as resources to present to your GP, and also to support a letter to your workplace/employer. This is all for documentation purposes, should it be needed as a reference point in any legal representation. They also double as an educational tool for both the GP, and the workplace/employer.
