A 54-year-old man is brought to the office by his friend because of worsening memory impairment over the past several months. The patient is also withdrawn and not focused during conversations. He is slow to answer questions or carry out instructions. The patient has a prolonged history of HIV infection and has had several opportunistic infections due to inconsistent antiretroviral medication use. On physical examination, he is alert and oriented. Muscle strength, deep tendon reflexes, and sensation are normal. MRI of the head shows diffuse cerebral atrophy with no focal mass lesions or areas of demyelination.

Answer E (correct answer): The question shows a Michaelis-Menten curve for an enzymatic reaction. Maximum velocity of enzymatic conversion of alcohol to acetaldehyde occurs at an ethanol concentration of about 0.08g/dl. At concentrations above this level, metabolism of ethanol into acetaldehyde will occur at a constant rate. Thus, when the blood alcohol level increases from 0.08 to O.lOg/dL there will be no change in rate of aceta!dehyde formation.

Answer C (correct answer): The graph is a Lineweaver Burk plot for the two enzymes, A and 8. Lineweaver Burk plots are a variant of Michaelis
Menten saturation curves that make the data linear rather than curved.

Answer B (correct answer): The Michaelis constant, Km, represents how avidly an enzyme binds to substrate. The lower the Km value, the more easily
the enzyme binds substrate. Systems with a low Km rapidly reach Vrnax because the enzyme is highly effective at binding substrate. Conversely,
enzymes with a high Km require large amounts of substrate to reach Vmax. ALDHC has a much lower Km compared to ALDHJ. Thus, ALDHC binds to
substrate with higher affinity.

So‘rovnoma
  •   A) Afinity of substrate binding to ALDHJ is higher
  •   B) ALDHC binds to substrate with higher affinity

  •   D) ALDHC enzyme has less rapid turnover

Answer B (correct answer): Km is the concentration of substrate when reaction velocity (V) is one half of Vmax. From the data table, Vrnax is
3.75mol/min. One half of Vmax is l.9mol/min. The data table shows a concentration of 0.Sg/ml at velocity of 2mol/min. Thus, Km is a number slightly
less than 0.5.
