❓ Does adherence to personal protective measures (e.g. hand hygiene or wearing of facemask) prevent pandemic diseases like influenza?

🔶 Developing a specific treatment or vaccine for a novel disease is a long and time consuming process, indicating the necessity of Personal protective measures as inexpensive method to prevent future pandemics in early stages

🔷 Meta-analyses of 16 studies suggest that:

✅ Regular hand hygiene provided a significant protective effect (OR = 0.62; 95% CI 0.52–0.73; I2 = 0%) against 2009 pandemic influenza infection
❎ Facemask use provided a non-significant protective effect (OR = 0.53; 95% CI 0.16–1.71; I2 = 48%) against 2009 pandemic influenza infection

🔴 Results suggest that campaigns to increase the frequency of hand hygiene, alongside use of facemasks in situations with a high risk of exposure, are likely to contribute to preventing pandemic influenza infection

⚖️ Which is More Important in Life: Love or Money ⁉️

🔶 As social beings, human behavior is strongly motivated by social rewards: adults purchase luxurious products to draw attention and admiration of others, adolescents devote a lot of time in video games for higher virtual social status, and children finish their homework with the expectation of being praised.

🔶 Money is also an effective reward and primary motivation that derives human actions.

🔷 Meta-analyses show that social and monetary reward anticipation engage a common neural circuit consisting of the ventral tegmental area, ventral striatum, anterior insula, and supplementary motor area.

🔴 In this manner, love is analogous to money in human brain.

Eggs: Healthy or Not ⁉️

🔶 Eggs are a major source of dietary cholesterol, but they are also an affordable source of high quality protein, iron, unsaturated fatty acids, phospholipids, and carotenoids.

🔷 Meta-analysis of three cohorts (>5.54 million person years) shows that consumption of at least one egg per day is NOT associated with risk of cardiovascular diseases

🔷 Meta-analysis of prospective cohort studies (1720108 participants) indicates that an increase of one egg per day is NOT associated with risk of cardiovascular diseases, coronary heart disease, and stroke

🔵 Meta-analyses stratified by geographical location make a very intersting point that up to one egg per day:

❎ is NOT associated with cardiovascular disease risk overall
✅ is associated with potentially LOWER cardiovascular disease risk in Asian populations

N95 respirator OR Surgical mask ⁉️

🔶 Conflicting recommendations exist related to which facial protection should be used by health care workers to prevent transmission of acute respiratory infections, including pandemic influenza.

🔷 Meta-analysis of 6 clinical studies (3 RCTs, 1 cohort study and 2 case–control studies) indicates NO difference between N95 respirators and surgical masks in risk of👇

❎ laboratory-confirmed respiratory infection
❎ influenza-like illness
❎ reported workplace absenteeism

🔷 Meta-analysis of 23 surrogate exposure studies reveals to us that under laboratory experimental conditions, N95 respirators are associated with less 👇

✅ filter penetration
✅ face-seal leakage
✅ total inward leakage

🦠Potential Therapeutics:

Apart from the timely diagnosis of cases, the achievement of favorable clinical outcomes depends on the timely treatment administered.

A number of novel compounds, as well as therapeutics, licensed for other conditions appear to have in vitro efficacy against the 2019-nCoV.

there are currently no effective specific antivirals or drug combinations supported by high-level evidence.

Potential Rapid Diagnostics, Vaccine and Therapeutics for 2019 Novel Coronavirus (2019-nCoV): A Systematic Review

https://www.mdpi.com/2077-0383/9/3/623

😒Since mid-December 2019 and as of early February 2020, the 2019 novel coronavirus (2019-nCoV) originating from Wuhan (Hubei Province, China) has infected over 25,000 laboratory-confirmed cases across 28 countries with about 500 deaths (a case-fatality rate of about 2%).

👨‍👩‍👦‍👦The 2019 novel coronavirus (2019-nCoV), a beta-coronavirus, forms a clade within the subgenus sarbecovirus of the Ortho-coronavirinae subfamily.

🏃Rapid diagnostics, vaccines and therapeutics are important interventions for the management of the 2019 novel coronavirus (2019-nCoV) outbreak.

📛⛔️Coronaviruses have error-prone RNA-dependent RNA polymerases (RdRP), which results in frequent mutations and recombination events.

Material and methods:

🔎 Search strategy:
An electronic search in Medline (PubMed interface), Scopus and Web of Science, using the keywords “procalcitonin” AND “2019 novel coronavirus” OR “2019-nCoV” OR “COVID-19” without date (i.e., up to March 3, 2020) and language restrictions

Inclusion criteria:
Documents reporting data in COVID-19 patients with or without severe disease (defined as needing admission to intensive care unit or use of mechanical ventilation).

Exclusion criteria:
Studies in which procalcitonin values were not reported in patients with or without severe COVID-19

⚠️ Procalcitonin values were entered as dichotomous variable, i.e., below or above the locally defined reference range (typically ≥0.50 ng/mL)

⚠️ Since the heterogeneity (I2 statistics) did not exceed 50%, a fixed effects model was finally used

Conclusion:
- Increased procalcitonin values are associated with a nearly 5-fold higher risk of severe SARS-CoV-2 infection (OR, 4.76; 95% CI, 2.74-8.29)
- The heterogeneity among the different studies was found to be modest (i.e., 34%)

To Migrate or Not To Migrate ⁉️

258 million people reside outside their country of birth; however, to date no global systematic reviews or meta-analyses of mortality data for these international migrants have been done.

🔷 A systematic review and meta-analysis published in Lancet aimed to review and synthesise available mortality data on international migrants.

✅ Meta-analysis has hsown that international migrants have a mortality advantage compared with general populations, and that this advantage persisted across the majority of ICD-10 disease categories.

✅ The mortality advantage identified will be representative of international migrants in high-income countries who are studying, working, or have joined family members in these countries.

✅ The results present an opportunity to reframe the public discourse on international migration and health in high-income countries.

🔵 Key messages

❎ Information is key; people who are quarantined need to understand the situation
❎ Effective and rapid communication is essential
❎ Supplies (both general and medical) need to be provided
❎ The quarantine period should be short and the duration should not be changed unless in extreme circumstances
❎ Most of the adverse effects come from the imposition of a restriction of liberty; voluntary quarantine is associated with less distress and fewer long-term complications
❎ Public health officials should emphasize the altruistic choice of self-isolating

#COVID19

⁉️The December, 2019 coronavirus disease outbreak has seen many countries ask people who have potentially come into contact with the infection to isolate themselves at home or in a dedicated quarantine facility. Decisions on how to apply quarantine should be based on the best available evidence. We did a Review of the psychological impact of quarantine using three electronic databases.

⚠️ Most reviewed studies reported negative psychological effects including post-traumatic stress symptoms, confusion, and anger.
⚠️ Stressors included longer quarantine duration, infection fears, frustration, boredom, inadequate supplies, inadequate information, financial loss, and stigma. Some researchers have suggested long-lasting effects.

🚫 In situations where quarantine is deemed necessary, officials should quarantine individuals for no longer than required.
❎ Appeals to altruism by reminding the public about the benefits of quarantine to wider society can be favourable.
