🔬 WHAT IS TISSUE PROCESSING?

🧠 Definition:
Tissue processing is the method of removing water from fixed tissue and replacing it with paraffin wax so the tissue becomes firm and can be cut into thin microscopic sections.


🎯 OBJECTIVES OF PROCESSING

🟢 Remove water from tissue
🟡 Replace water with embedding medium
🔵 Make tissue firm for sectioning
🟣 Preserve tissue morphology
🔴 Prepare tissue for uniform staining
⭐ Exam clue:
No processing → No section → No diagnosis
⚙️ MAIN STEPS OF TISSUE PROCESSING
🟡 1. DEHYDRATION
💡 Removes water from tissue
🧴 Reagents:
70% alcohol → 80% → 95% → Absolute alcohol
🧠 Principle:
Gradual replacement prevents tissue distortion.
⚠️ Too rapid dehydration → Tissue shrinkage

🔵 2. CLEARING

💡 Removes alcohol & prepares tissue for wax
🧴 Agents:
✔ Xylene ⭐ (Most common)
✔ Toluene
✔ Benzene
✔ Chloroform
👁️ Tissue becomes transparent
📌 Exam favorite:
Clearing agent = Xylene

🔴 3. INFILTRATION

💡 Wax enters tissue spaces
🔥 Medium: Paraffin wax
🌡 Temperature: 56–60°C
🧠 Wax replaces clearing agent and gives strength.
⚠️ Overheated wax → Tissue becomes hard
🟤 4. EMBEDDING
💡 Tissue is placed in mold and wax solidifies.
🎯 Importance:
Correct orientation ensures correct diagnosis.
📌 Example:
Epithelium must be cut perpendicular to surface.
👉 Final product = Paraffin block
🧱 EMBEDDING MEDIA
🟡 Paraffin wax → Most used
🔵 Water-soluble wax → Special cases
🟣 Plastic resin → Electron microscopy
⭐ MCQ tip:
Most common = Paraffin wax
⚠️ FACTORS AFFECTING QUALITY
📏 Tissue thickness (ideal 3–5 mm)
🧪 Proper fixation first
⏱ Correct timing
🧴 Reagent purity
🔥 Correct wax temperature
⭐ Golden rule:
Bad fixation → Bad processing
❌ ARTEFACTS FROM POOR PROCESSING
🔻 Tissue shrinkage
🔻 Poor infiltration
🔻 Tissue too hard
🔻 Distorted orientation
👉 These lead to wrong diagnosis

The Bilirubin test is a biochemical blood test used to measure the level of bilirubin in the blood. Bilirubin is a yellow pigment formed from the breakdown of hemoglobin in red blood cells.

🔹 Types of Bilirubin:
• Total Bilirubin
• Direct (Conjugated) Bilirubin
• Indirect (Unconjugated) Bilirubin

🔹 Clinical Use / When doctor advises this test:
This test is commonly advised in patients with jaundice, liver disorders, bile duct obstruction, and hemolytic conditions. It helps in differentiating hepatic, pre-hepatic, and post-hepatic jaundice.

🔹 Sample Required:
🩸 Serum

🔹 Principle (in simple words):
Bilirubin reacts with a diazo reagent to form a colored compound.
The intensity of the color produced is measured and is directly proportional to the bilirubin concentration in the sample.

🔹 Normal Values:
• Total Bilirubin: 0.3 – 1.2 mg/dL
• Direct Bilirubin: 0 – 0.3 mg/dL
(Values may vary slightly between laboratories)

🔹 Increased Bilirubin Levels seen in:
• Jaundice
• Hepatitis
• Liver cirrhosis
• Hemolytic anemia
• Bile duct obstruction

🔹 Clinical Importance :
Bilirubin test is an important part of Liver Function Tests (LFT) and helps clinicians understand liver function and red cell breakdown.

🔬 1. BACTERIAL STRUCTURE & FUNCTION


✅ Basic structure

✓Cell wall → shape, protection, Gram reaction
✓Cell membrane → transport, respiration, enzyme activity
✓Cytoplasm → metabolic reactions
✓Ribosome (70S) → protein synthesis
✓Nucleoid → bacterial DNA (single circular chromosome)

✅ External structures

✓Capsule → virulence, anti-phagocytic
✓Flagella → motility
✓Pili/fimbriae → attachment, conjugation
✓Spore → survival in harsh conditions
📌 Exam key: Capsule = virulence, Ribosome = drug target, Spore = resistance
🧬 2. PLASMID
✔ Small extra-chromosomal circular DNA
✔ Replicates independently
✔ Carries antibiotic resistance genes
✔ Can transfer between bacteria
📌 Exam key: plasmid = drug resistance + gene transfer
🧬 3. BACTERIAL GENETICS
✔ Mutation
Permanent DNA change
May cause drug resistance
✔ Gene transfer methods

🔄 4. GENE TRANSFER MECHANISMS
🧪 Conjugation
✔ DNA transfer via sex pilus

🧪 Transformation
✔ Uptake of free DNA from environment
✔ No cell contact required
📌 Seen in: Pneumococcus, Neisseria
🧪 Transduction
✔ DNA transfer via bacteriophage (virus)
📌 Phage carries DNA from one bacterium to another
🦠 5. BACTERIOPHAGE
✔ Virus that infects bacteria
✔ Injects DNA into bacteria
Types:
Lytic cycle → bacterial destruction
Lysogenic cycle → DNA integrates into host
📌 Exam key: phage = gene transfer + toxin production
